Dr. Vildozola's orthopedic practice closed on May 18, 2004. His practice was located at 62 Buckland Street, Suite 201, Manchester Connecticut. The State Department of Public Health has been working for the past two years to make patient medical records available to his former patients.

Since July 17, 2006, the Manchester Health Department has been assisting the State Department of Public Health, Medical Records Division, by serving as a local distribution point for past patients to pick up personal medical records. Following a notification campaign that included newspaper notices, individual mailings and some phone calls to attempt to contact past patients, the opportunity to retrieve personal medical records will end this Friday, September 15, 2006. All remaining medical records will be destroyed September 21, 2006.

Anyone who is a past patient of Dr. Vildozola and is still interested in retrieving personal medical records must contact the Manchester Health Department at (860) 647-3179 IMMEDIATELY to schedule an appointment for personal pick-up of medical records. Please leave a voice mail message and someone will call you back as soon as possible. No records will be mailed due to HIPAA Law restrictions. Medical Records are available for pick up Monday through Friday, between 8:00 a.m. and 4:30 p.m. at the Manchester Health Department, Weiss Center at 479 Main Street (the former Post Office), follow signs to the Health Department on the first floor.
