If you are Party 1 or Party 2,  you must  include a photocopy of your driver's license, state issued non-driver photo ID or passport.

Civil Union  licenses are public information. Anyone can get a certified copy of a Civil Union license. If you are not Party 1 or Party 2 you may still get a certified copy of a Civil Union certificate, but you will receive a certified copy with the social security numbers removed.

If you are requesting a copy of a civil union certificate, and the ceremony  did not occur in Manchester, or the persons involved did not live in Manchester at the time of the ceremony, please send this request to the town in which the ceremony took place.
