Medical records available for past patients of Dr. Carlos Vildozola.

Dr. Vildozola closed his practice on or about May 18, 2004. He had an orthopedic practice at 62 Buckland Street, Suite 201 in Manchester for many years. The State Medical Records Division secured possession of his medical files, and only records from the last seven years of his practice will be available (1997 to 2004).

Anyone interested in retrieving medical records for themself, their child or another person (with a signed permission note from the patient that includes the past patient's Date of Birth) is being instructed to phone the State Department of Public Health at (860) 509-8290, and leave their full name, Date of Birth, and the day and time they will be in to collect their medical records. Hour are from 9:00 AM to 4:00PM, Monday through Friday until August 11, 2006. Medical records will then be ready for pick up. Again, only records from 1997 to 2004 are available.

The medical records are being distributed from the Manchester Health Department office, in Weiss Center (former Post Office Building) at the corner of Main and East Center Streets. Parking is available behind Lincoln Center across the street.

All records that are unclaimed by September 17, 2006 will be destroyed.
